- Schedule your appointment by completing the form below or calling our office directly.
- Prepare Your Device
- If using a mobile device, download the Zoom or Microsoft Teams app.
- if using a laptop or desktop, access the visit through Google Chrome for the best performance.
- Ensure your microphone, camera, and speakers are enabled and functioning properly.
- Check-In Process (Prior to Appointment)
You’ll receive a text or email with instructions to:
- Complete Patient Forms
- Check-in Online
- Confirm Technology Readiness
- Submit Consent Forms
- Verify Insurance
- Make Your Copay or Self-Pay Payment
- Join the Virtual Visit
- If your visit is about to begin, you may enter the virtual waiting room immediately.
- If not, you will receive another email with a link to join your session.
- Provide Additional Details
- Enter your callback number and preferred pharmacy so your provider has accurate, updated information.
- Start the Visit
- When your provider is ready, an alert sound will notify you.
- The “Join Visit” button will become active—click to begin your session.
- Your provider will appear on the screen shortly.
- After the Appointment
- You and your provider will end the session once the visit is complete.
- Any next steps or instructions discussed will be documented and managed accordingly.
- A survey invitation will be sent to share feedback about your experience.
